Welcome to the Garden. This is your access point for client forms, questionnaires, and appointments related to our holistic wellness offerings.
Getting Started
New Clients: Before your first session, please download and complete the Intake, Pain Mapping, and Liability Waiver forms. These are essential for your journey toward somatic restoration services.
Current Clients: If you have not done so, please complete the Follow-Up and Waiver forms. Prenatal clients should fill out the Physician's Clearance as well.
Submission: Please email all completed client forms to master.vermont@gmail.com or bring them to your appointment. We're here to help you achieve your wellness goals.
Our Appointments
To maintain structural integrity and focus, no walk-ins are permitted. Established clients requiring emergency restoration should reach out to Master V through their private contact line. For those interested in holistic wellness, remember to complete the necessary client forms to access our somatic restoration services.
Client Requirements & Policies
Please review these protocols before downloading your client forms for intake paperwork.
Policy & Access: To maintain structural integrity and focus, no walk-ins are permitted. We are an appointment-only practice dedicated to holistic wellness.
24-Hour Cancellation: Cancellations with less than 24 hours' notice will be charged the full service fee. This ensures the sustainability of the practice and respect for the scheduled time.
Emergencies: Established clients requiring emergency restoration services should reach out to Master V directly via your private contact line.
HIPAA Compliance & Data Security: Your health privacy is a priority. We maintain HIPAA-compliant standards for the handling of all clinical records and personal health information (PHI).
Secure Storage: All digital intake forms and "Internal Matrix" notes are stored on encrypted, password-protected platforms.
Confidentiality: Your clinical data is never shared with third parties without your written authorization, except as required by law for your safety.
For maximum security, you may also bring your completed physical forms to your first session rather than emailing them.
